PLEASE RETAIN YOUR FITTING CARD AND ORIGINAL TILL RECEIPT TO CLAIM

1.
     This is a Consumer only promotion for clubs fitted and purchased at current Mizuno Performance Centres, as identified on Mizuno Golf website http://golf.mizunoeurope.com/

2.     Clubs purchased must be custom fit and must match those recommended by Mizuno Swing DNA software at selected Mizuno Performance Centres.

3.     To qualify for the guarantee clubs must be purchased at the Mizuno Performance Centre where they were fitted.

4.     You must retain your original valid till receipt and the Swing DNA Fitting Card you were supplied with during the fitting.

5.     Claims can only be made between 6 and 8 weeks after the date of purchase by contacting the Mizuno Performance Centre where you were fitted and where you made your purchase.

6.     If your claim is valid you will be refitted and supplied with alternative Mizuno clubs.

7.    There is no cash alternative.

8.     Claims made without a valid original till receipt and Swing DNA Fitting Card will not be accepted.

9.     One claim per household, 16+ only, normal exclusions apply.
